Background
The MHSPHP CarePoint is a tool to assist the Medical Treatment Facilities (MTFs) actively manage the health of their enrolled patient populations by comparing their performance on select HEDIS measures with established benchmarks. The MHSPHP CarePoint provides both patient level and aggregate level data.
The PHN Dashboard presents aggregate data on all MTFs Navy-wide and over time without having to establish an MHSPHP CarePoint account. The PHN Dashboard graphs are composed from the same data used in MHSPHP CarePoint.
