What are the current requirements for the Post Deployment Health Assessment and Post Deployment Health Reassessment for the Navy and Marine Corps?
Answer: DODI 6490.03 of August 2006

To summarize the requirements include:

  1. Completion of the four-page, Revised DD Form 2796 Post-Deployment Questionnaire
  2. A face-to-face health assessment with a health care provider
  3. A blood sample from all redeploying personnel
  4. Completion of the DD form 2900, ninety to 180 days after return from deployment

What pre-deployment medical screening and other processes are applicable to civilian contract personnel?
Answer: The Department of Defense Instruction (DODI 3020.37, Nov 6, 1990), Continuation of Essential Contractor Services.  Enclosure 3, provides detailed information on what services may be provided to contractors who deploy in support of military operations.  The patient category for contractors is K65, while the FMP is 99+ssn.   Services can include receiving pre-deployment environmental threat briefings, immunizations, panarex films, HIV testing, and medical evaluations (medical, dental, or psychological) for fitness to deploy.

Where do we send the blood samples?
Answer: Please look up the May 2003 BUMED Guidance

Draw blood samples as HIV draw and process as such.For afloat units, please contact the nearest MTF laboratory to coordinate delivery and processing.  Please see additional comments below from CAPT Kilbane.

Why do we draw HIV blood samples since getting HIV on cruise seems unlikely?

Answer:This takes care of two requirements, i.e., specimen from troops redeploying from Operation Iraqi Freedom as well as to have the HIV tests done within 30 days of redeployment.

Can medical staff aboard a ship conduct the Post-Deployment blood testing?
Answer: Medical staff can register with the Navy HIV program as an authorized representative by e-mailing the Navy Central HIV Program at nchp@bethesda.med.navy.mil  if they want to conduct their own processing of blood for post-deployment. As an alternative ships may also coordinate with MTFs for assistance in drawing these specimen.

Are post-deployment blood specimens identified as to the reason for testing being conducted?
Answer:A "source of test" code "H" is used for redeployment testing when test data is uploaded from SAMS. If the tests are submitted through CHCS, the source of test code "H" is not available. "F" or "P" may be used. Likewise, the Medical Readiness Reporting system (MRRS) does not yet allow for entry of the "H" code. When the SOT "H" is used, the specimen is sent to Viromed but not tested for HIV at that time. If not marked with an "H", an HIV test is run and the results returned to the facility that ordered them. Test results from SAMS are sent by VIROMED to the Navy HIV Center in Bethesda.

How do I complete my Post Deployment Health Assessment (DD2796) if our command does not have access to the Internet in theater?

Answer:  Contact the edha help desk or by phone at 757-953-0717 (DSN 377-0717) Staff will provide you with an Access-based program to complete the post-deployment health assessment (DD2796).

I am a RESERVIST or NATIONAL GUARD MEMBER. How do I complete my 3-6 month Post Deployment Health Reassessment (DD2900)if I do not have access to the Internet or to a military health care provider authorized to review my survey?

Answer: Reservists and National Guard members can call Logistics Health, Inc. tool-free at 888-734-7299 and complete the survey verbally over the phone. A physician will review your responses and prescribe appropriate follow up care.

I see a civilian healthcare provider because no military treatment facility is located close to where I am assigned (e.g., recruiting duty). How do I complete my Post Deployment Health Reassessment form (DD2900)?

Answer:The Navy is currently addressing this problem. In the meantime, contact PostDep@nehc.med.navy.mil for personal assistance. Please state your situation and provide a contact phone number.

I am new to the EDHA system. What kind of access do I need?

Answer: There are 3 kinds of accounts: Local Administrator, Provider, and User. A person may also have both Local Administrator and Provider accounts.

The Local Administrator or medical program manager and has the ability to:

  • Add Providers and reset passwords for Users
  • Set/Distribute Passphrases
  • Generate Survey Status Reports

The Provider or Independent Duty Corpsman has the ability to:

  • Retrieve User Surveys
  • Certify User Surveys
  • Print User Surveys

The User or Sailor/Marine may:

  • Create/Submit/Print Surveys

I am new to the EDHA system. I click on the "New User?" button and it prompts me for a passphrase. Where do I get a passphrase?
Answer:The “New User?” button is for service members new to the system that need to register and fill out their health forms. It is not for Local Administrators, medical program managers, or Providers. Contact your Local Administrator for a passphrase, but be aware that you will not be given one unless it is within 72 hours of your have a scheduled medical appointment.

I am a service member trying to complete my health form online. I can't remember my login or password. I also can't remember the answer to my security question. What do I do?
Answer:Your login is your social security number. Your password may be reset by your Local Administrator.

I am the medical program manager for EDHA and need Local Administrator access. Where do I go to get a login/password or passphrase?
Answer: You do not need a passphrase. To get a login/password, email the EDHA Helpdesk at edha@nehc.mar.med.navy.mil

I am a Local Administrator. How do I create a Provider account?
Answer: After logging into EDHA, you would select “Manager Users” and fill in the form. Or you may email the EDHA Helpdesk at edha@nehc.mar.med.navy.mil

I am a Local Administrator. How do I create a User account?
Answer:You don't. You should create a passphrase and distribute your passphrase only to Users who have a scheduled medical appointment within 72 hours. You can create a passphrase to last from 2 to 24 hours. The User should click the “New User?” button and use the passphrase given to them to create their own accounts.

Where are the hard copies of the deployment health screening forms filed in the medical record?
Answer: According to BUMED (M3B12), Patient Administration and Tricare Operations, completed hard copy DD 2795, DD 2796, and DD 2900 forms should be filed in Part 3 of the Navy-Marine Corps medical record. This is the same section that holds History and Physical exams (H & Ps) and Overseas Screening forms. Guidance on the organization of the hard copy medical record can by found in Chapter 16 of the MANMED, however, please be advised that this chapter is undergoing review and updating at present.
