A DVD/CD product that can support prevention counseling of members during the Periodic Health Assessment, can decrease risk behaviors among patients in disease management clinics, or can promote healthier lifestyle behaviors during any primary care encounter is available from the Navy and Marine Corps Public Health Center (NMCPHC) and the Visual Information Directorate at Navy Medicine Support Command (NMSC). This product is appropriate for all levels of providers who wish to learn how to effectively and efficiently implement a counseling process to get patients to adopt healthier lifestyle behaviors and to become more engaged in their personal healthcare.

Ordering Instructions

  1. Log on to the Department of Defense Imagery website
  2. In the gray search box, check “A/V and IMI Products.”
  3. Uncheck “still images,” “motion imagery” and “site content.” Enter the search PIN number 808850 in the Search box and click on SEARCH.
  4. Click on the title.
  5. Click “Add Production to Cart” to order and click “OK.”
  6. Click “SHOPPING CART” in the top right corner to view your Shopping Cart and quantity requested.
  7. Click STEP 2 and fill out the order form. Use the additional Instructions block for any special requests with justification if you are ordering multiple copies or requiring Fedex delivery. You need to provide your complete Military Address along with your .mil email address to process your order.
  8. Complete the process by confirming your order.
