If your command provides industrial hygiene (IH) services to Navy, i.e., USS, and Military Sealift Command (MSC), i.e., USNS, ships, you should be aware that BUMED tracks which ships have Baseline Industrial Hygiene Surveys and current Periodic Industrial Hygiene Surveys. This is to ensure that Force Health Protection is receiving the high priority it has been assigned. A survey as considered complete, if it is in the standardized format outlined in Chapter 14 of the Industrial Hygiene Field Operations Manual (IHFOM).
An Excel spreadsheet file with the names of Navy and MSC ships listed in alphabetical order is provided to allow IH Program Offices to see what IH surveys are available. This allows IH Program Offices to easily obtain a copy of the current survey when a ship changes homeport or an MSC ship is requesting a survey at a location other than their usual homeport. The spreadsheet contains the date of the Baseline IH Survey and all Periodic IH Surveys held by the Navy and Marine Corps Public Health Center (NMCPHC). All survey dates are the date on the cover letter that transmitted the survey report. The spreadsheet reflects only those surveys that have been sent to NMCPHC by the IH Program Office so a survey will not appear in the spreadsheet if a copy was not sent to NMCPHC.
